Please read this statement before you provide any personal information to the University Health Service @ Sassoon Road (“UHS@SR”).

 

1.  Purpose of Collection

Our staff members and healthcare providers may ask you to provide personal data (including, but not limited to, your name, gender, date of birth, identification document type and number, home address, email address, emergency contact person’s name and contact details, your health condition, past medical history, and medication history).

 

​UHS@SR may also obtain from any appropriate third party your medical history or relevant information. Such information is for purposes relating to your healthcare or treatment related purposes or purposes directly related to any of them including verification of your identity, appointment booking and registration and/or communication with you in relation to arrangement or provision of services, handling matters related to payment and/or insurance claims, investigation and processing of enquiries or complaints, clinical statistics and audit.

 

​The provision of your personal data is voluntary. However, if you fail to provide any required or accurate and complete personal data, it may affect our ability to provide services to you.

 

 2.  Provision, Transfer and Disclosure of Personal Data

The personal data supplied by you may be transferred to parties who will handle, and/or who may be contacted by us during the handling of, this activity, for the provision of services to you under the care of the UHS@SR, including our staff, and/or any appropriate third parties as we are required by law or regulation for the purposes stated in this statement.

 

Since 1 September 2026, UHS@SR has joined the Medical Network of AIA International Limited (AIA) as one of its network GP clinics. Eligible staff and their dependants who visit UHS@SR will receive medical services as members of AIA’s Group Medical Insurance scheme. Under this arrangement, UHS@SR is required to submit and transfer your personal data and medical records to AIA and its wholly owned subsidiary, U Care Medical Hong Kong Limited, to facilitate reimbursement claims after each of your visit. Please note that this arrangement is not applicable to eligible students and retirees who are not covered under the said medical benefits scheme.

The personal data supplied by you may be made available to clinical teachers and students at the University of Hong Kong, whose presence at UHS@SR are authorized for the purpose of on-site clinical teaching activities.

 

The personal data supplied by you, upon anonymization to remove any personally identifiable information, may be made available to researchers at the University of Hong Kong for the purpose of research.

 

Unless you have given prior consent, the UHS@SR will not disclose your personal data to any third party for purposes other than those stated above or directly related purposes.
